Qualification Process

Qualification for the Arizona Medical Marijuana Program requires a strict certification process. While we must adhere to Arizona law in our duties, we endeavor to make the medical marijuana application process as quick and easy as possible for our patients.
Following are the steps in the qualification process.

Step 1: Medical Record Collection

The first step in the Medical Marijuana application process is documentation of your medical condition. You must have a prior diagnosis with one of the conditions above. This means that you must have a chart note from a healthcare provider proving your diagnosis, and mentioning the treatment that was offered. Without current and verifiable medical records, our physicians will be unable to certify your condition under Arizona law. Although there are conditions where a patient will qualify for the Arizona Medical Marijuana Program without the necessity of full documentation, the Arizona Medical Marijuana Card Certification Center (AZMMCCC) requires that  your record is current within the last 12 months and documents your qualifying condition.

We must have this paperwork from your healthcare provider during or before the visit. However, if you have chronic pain, we can schedule a same day chronic pain evaluation prior to the visit.

If you prefer that we handle your medical record collection, you can complete the medical release form and fax or mail it to our office so that we may request your medical records from your treatment providers. If you already have your medical records, you can get this paperwork to us in any of four ways:

  1. Upload it in our intake form (preferred) OR

  4. Just bring it to your visit with the doctor.

Step 2: Physician Review

Upon receipt of your medical records, our physicians thoroughly review the records to determine if you qualify for the AZ Medical Marijuana Program. If additional documents are required for the review, a AZMMCCC staff member will contact you and let you know which documents are needed for our records. If you qualify for the program, we will call you to schedule an appointment with one of our physicians.

Step 3: Certification Appointment

At your certification appointment, our physicians will talk with you about your qualification for certification of a medical condition that may qualify you for the Arizona Medical Marijuana Program. By law, our physicians must only discuss your qualification for the program. After review and approval, qualified patients will receive a signed Physician Certification Form that will allow them to apply to the state for a Medical Marijuana Identification Card. In order to receive the Physician Certification Form, patients must present a valid Arizona ID, such as a Arizona ID or Arizona driver’s license, along with the physician visit fee. Please note that insurance companies do not cover medical marijuana consultations or the state’s medical marijuana application fee.

Step 4: Application for AZ Medical Marijuana Identification Card

In order to obtain your Medical Marijuana Identification Card, you or we must complete an application with the Arizona Department of Health Services. This application includes the following information:
